Monica Alvarez, MBA

Hello, I came to this company with almost 40 years of healthcare management experience. This journey was based on an unwavering passion for making a difference in the lives and individuals in need. I have spent the last 15 years as the Chief Operations Officer with CORE Physical Therapy, an outpatient physical therapy corporation that services the needs of Los Angeles, Orange, and Riverside County communities. Today, as the CEO of Van Doc, HC DBA C.O.R.E At Home, I have the privilege of partnering with a dedicated team of caregivers. The genuine commitment and compassion shown by our providers to our patients, surpasses our deep desire for the patients entrusted to our care. We look forward to the opportunity to make a difference for you or any family member referred to our services. I wish you all continued health and good fortune.

Carey Laurent, PT, DPT

Dr. Laurent was born and raised in La Habra, CA with two older sisters, an older brother, and two loving parents. From a young age, he played any and all sports, and took a liking to basketball and soccer the most. He attended Servite High School in Anaheim, CA, where he played basketball for 4 years and lacrosse for 1 year. He went on to graduate from UC Santa Cruz, home of the Banana Slugs, with a BS in Health Sciences.

It was during his undergraduate years, he found that physical therapy was his calling. After undergraduate studies, he decided to move to the South for 3 years and attend the Doctor of Physical Therapy school at Western Kentucky University, where he was class treasurer in charge of fundraisers. Dr. Laurent has been working at CORE PT since September 2018 and enjoying every minute of it.

Dr. Laurent enjoys treating all types of post-operative and acute diagnoses, especially those involving spinal procedures and hip/knee replacements. He gets ultimate satisfaction when he sees his patients get better each day and reach their personal health goals. Dr. Laurent serves as an APTA-credentialed clinical instructor here at CORE Physical Therapy

Over the years, he has increased his grasp of the Spanish language and is now independent when working with Spanish speakers here in the clinic. During his free time, Dr. Laurent enjoys doing anything outdoors, whether this is shooting hoops, going to the beach, kayaking, going on hikes, or traveling.

Ming Chiu, PT, DPT

Dr. Chiu received his Doctorate in Physical Therapy from Loma Linda University.  He learned from his early teenage years from his recreational sports injuries how Physical Therapy is an integral part of his success to resume the sports he enjoys, and how it can also improve his quality of life.  As a Physical Therapist who specializes in the science of human body movements, Dr. Chiu not only incorporates manual skills but also includes proper biomechanics along with neuromuscular control to achieve long-term goals of the patient to have a better quality of life.  In his free time, Dr. Chiu spends his free time volunteering as Little League Umpire to build a strong foundation of sportsmanship, and life lessons in the community for the young children. 

Tyler Rooze, DNP, MSN-Ed., MPH, RN, PHN
Director of Patient Care Services – Home Health Division

Dr. Rooze was born and raised in Southern California. At an early age, he developed a passion for caring for others in need. This calling initiated his lifelong journey into the field of nursing. He attended West Coast University in Ontario, CA where he graduated with a B.S. in nursing, eventually pursuing a master’s degree in public health, a master’s degree in nursing education, and most recently, his doctoral degree in nursing and executive healthcare leadership. As a registered nurse, he has worked in various community-based organizations that provide rehabilitative care to various unique patient populations.

 

As a board-certified public health nurse, Dr. Rooze assists patients in their goal of achieving an optimal health level. He believes that all patients can improve their health, with the proper support from dedicated and committed clinicians. As a healthcare leader, Dr. Rooze works to develop meaningful and targeted interventions that drive patient health.

 

In his free time, Dr. Rooze is an adjunct nursing professor for several prominent universities in Southern California. He also enjoys spending time with his loving wife and three amazing children. Aside from nursing, teaching, and family, Dr. Rooze is an avid chef and enjoys experimenting in the kitchen.

Krista Contreras, PT, DPT

Dr. Contreras is a board-certified Doctor of Physical Therapy. She graduated from the University of Southern California in 2016 where she earned her Doctorate in Physical Therapy. She was born and raised in Orange County. She attended Rosary High School in Fullerton, where she played tennis and softball. She then started her pursuit of a career in Physical Therapy, a lifelong dream of hers, by earning her bachelor’s degree at Loyola Marymount University.

 

With over eight years of experience, Dr. Contreras has developed a special interest in treating diagnoses including neurological disorders, balance and gait impairments, and supporting those recovering from orthopedic surgeries.

 

Dr. Contreras is truly invested in the success of her patients and dedicates herself to developing specific therapeutic interventions to optimize a patient's function in daily living, minimize pain, and restore mobility.

 

Dr. Contreras has firm beliefs in the value of evidence-based physical therapy treatment and strives to provide exceptional care to all of her patients. When she is not treating patients, she is usually still reading research articles about Physical Therapy and taking continuing education courses. Dr. Contreras enjoys cheering on her two children at their various sporting events with her husband and family, swimming, biking, watching Angels baseball games, and going to theme parks.

Matthew Reekstin PT, MPT, MBA

Matthew Reekstin PT, MPT, MBA has been practicing physical therapy in Orange County for the past 23 years and is the President and Founder of C.O.R.E. Physical Therapy. Over the years, C.O.R.E Physical Therapy has grown to 4 locations and is well known for its quality and highly reputable outpatient physical therapy services. The need for the same type of exemplary service has grown in the home health sector in recent years, and Mr. Reekstin recognized this need by forming a team of professionals that best embodied the traits necessary to bring C.O.R.E. Physical Therapy’s values to the home. Thus, C.O.R.E. at Home was born. Mr. Reekstin and his team look forward to serving the same community they have already served for over 20 years, for many years to come. With a combined experience of over 100 years, the owners of C.O.R.E. At Home pledge to provide the best that in-home physical therapy has to offer. 

 

On a personal note, Mr. Reekstin lives in Fullerton, CA with his wife and two daughters. He supports the community as a Member and Past President of Fullerton Rotary and has been heavily involved with local sports, especially with his two daughters’ water polo teams. He enjoys playing and watching most any sport, and golfs and skis when given the opportunity. He grew up in Orange, CA, and was a multi-sport athlete at Villa Park High School. He graduated from the University of Arizona with a Bachelor’s in Health Science and then went on to receive two Master’s Degrees, an MBA from Chapman University, and a Master’s in Physical Therapy from Western University of Health Sciences. 

Kellie Redding- Administrator

For the past 35 years, I have worked in various areas of healthcare management and direct patient care. My career started after college and professional training as a certified surgical assistant. After almost two decades in patient care, I took time away to spend time with my family and raise my beautiful daughter. Since returning to the healthcare arena, I have been working in the opposite area of HR and management. Now with my full range of understanding for both patients, providers, and referring providers, I am proud to be associated with this amazing company and group of partners. Having worked with these types of services for my family members in the past, I know how to treat our patients like family. We are here to treat everyone with care and respect.
